Required paperwork

These are the REQUIRED FORMS for each student, that must be completed and submitted every school year.

  1. Acknowledgement Form
  2. FCPS Emergency Care Form
  3. FCPS Health Form
  4. Health Screening Parent/Guardian Commitment Form
  5. Transportation Agreement Form - and please read this Transportation Info 

Optional: If your student will be driving to Fairfax Academy, they must submit a Parking Tag form, and include a copy of their driver's license or permit.
