Required paperwork
These are the REQUIRED FORMS for each student, that must be completed and submitted every school year.
- Acknowledgement Form
- FCPS Emergency Care Form
- FCPS Health Form
- Health Screening Parent/Guardian Commitment Form
- Transportation Agreement Form - and please read this Transportation Info
Optional: If your student will be driving to Fairfax Academy, they must submit a Parking Tag form, and include a copy of their driver's license or permit.
